The questions in the assessment are similar to those asked during the Initial Consultation to determine whether the Davis programme is able to help the individual to reach their goals.

There are no right or wrong answers, just answer honestly or use your judgement if you are completing the assessment for someone else. It has five pages that list various characteristics - select one of the three options for each item. You can skip any questions that don't apply.

When completed, you can print the assessment results for future reference and/or bring with you to a consultation.
